There are many ways to deal with thick nasal tip skin, and if somebody were to develop a poorly healed wound on the tip of their nose, it may make the entirety of the operation not worth it. Hello and thank you for your question. Placing an incision directly in the nasal tip is not a good idea. It often times results in an indented scar with loss of tissue and poor shape. Do not judge any techniques based on one result. You should be able to see hundreds and hundreds of longterm photos on your surgeon’s website with proven reliable techniques. Although the result you show in that patient is very good I don’t think the skin excision was a good idea or helped in the overall result. I would not have excused skin in the way those photos show

Since rhinoplasty is the more delicate of the two surgeries, it should be performed first. If the facelift is performed first, it could lead to unnecessary nasal swelling that could be distracting for the rhinoplasty surgeon. I hope this helps! Sincerely, Dr Joseph

Since somebody's nose is part of their Airway, invariably, there is bleeding from the nose into the airway. Having general anesthesia delivered through an endotracheal tube may prevent blood and secretions from entering the airway during rhinoplasty surgery. Additionally, I do not think it is...

In your before photo, you appear to have more of a forced smile than in your after photo which makes your nasolabial folds appear deeper before surgery. In that regard, I feel like your nasal labial folds may appear better after surgery, or perhaps less deep. I hope this is helpful and thank you...
