I had a closed revision rhinoplasty im 4 month now I know that i'm still healing, my rhinoplasty went good i like my nose despite the short columella I hate it and I want to lengthen it My question is can i do it after the 6 month mark in closed surgery again or is there any chance of necrosis? And what is the best method ? Ps: i had a crooked nose with hanging columella after my primary surgery
Answer: Revision Rhinoplasty Hello! Thank you for your question.I have checked your case from the photos you provided.Your nose shape looks great overall. Anyway as you mentioned you have a hanging columella that can be corrected (a few mm). You will most probably need rib cartilage reconstruction. I recommend you to wait at least 1 year after your last surgery to get a new revision.Best regards!

Answer: Things you should know about revision rhinoplasty Thank you for your question and photos. It is possible to lengthen the columella during revision rhinoplasty, though the technique and type of procedure would need to be determined with an in-person evaluation. Your nose will continue to recover for at least one year after your initial surgery, so it is recommended to wait until this time to assess your final results and consider revision. Before selecting revision rhinoplasty, please note that each subsequent nose surgery becomes more complex due to the loss of structural support, scar tissue, and other factors. The importance of experience, expertise, and artistry cannot be overstated. I recommend scheduling an in-person consultation with an experienced, board-certified facial plastic surgeon, with expertise in rhinoplasty techniques. They will be able to review your surgical history, assess your concern, and determine the best approach for you.

Answer: The method depends on the overall outcome you want. If you want a natural and small change of the columella, surgery is possible through cartilage graft with a closed approach, and in that case, it is judged that there is little concern about necrosis. However, if you want to make some big changes, you need an open approach. In-person consultation and examination are needed to determine what level of change should be aimed at.

Answer: I want to lengthen my columella, what is the best method & can I do it at 6 month-mark? Hello @Sbsb123, thank you for your question. It is important to wait at least 6 months after your initial rhinoplasty before considering any additional surgical procedures in order to allow the tissue to heal properly. After such a time frame, you may consult with your specialist and determine whether or not a revision rhinoplasty would be beneficial in this case. Each case is unique, and only a board-certified plastic surgeon who has examined you and knows your medical history can provide accurate advice on how to proceed. The techniques that might be used are cartilage grafting or fillers that are a less invasive an permanent solution. The optimal procedure for you depends on the particulars of your nose, prior operations, and your own personal goals. A consultation is vital to establishing a surgical plan that meets your needs, expectations, and takes into account what is realistically possible. Best wishes! Alan Gonzalez MD, FACS. Answer: The appearance of your Columella may improve as your nasal tip swelling goes down. The appearance of slight columella retraction could be due to the fact that your tip is still somewhat swollen. It's the appearance were to remain as it is, a few droplets of Silikon-1000 in the mid Columella may be helpful for achieving a straighter columella appearance on profile. If this is your only concern about your nasal appearance after rhinoplasty, I would not recommend further surgery. Hope this helps! Dr Joseph
