I had my first consultation on April 30. After my first appointment, he advised that my breasts are "droopy and have a bit of a sag to them". His recommendation would be 350 min to 400ccs if not more. He indicated that i have very little breast tissue and would need volume to lift the sag not to mention to go under the muscle for that same reason. I have posted a few photos of my current look and desires. I would like them to be proportionate to my body. Any advise would do. Thank you.
Answer: The most proportionate breast size. Your picture doesn't show any droopiness, but you do have adequate skin to accommodate a 300 to 400 cc implant. I usually give the patients a range and then we try these different sizes in appropriate bras. I find that this is the best way for the patient to visualize what they want. As far as proportion I consider a buttocks to breast ration that I have written about in the past. You have the luxury of having an appropriate breast shape and soft tissues to allow for a wider range of implant sizes. You should expect a very good result.

Proper sizing is not just about the number of cc’s. The thickness of your tissue, breast dimensions which include the width, height, and projection, as well as chest wall width all need to be considered when choosing an implant. Trying on implant “sizers” of various shapes and volumes while wearing a tight t-shirt, bra, or bathing sit at a preoperative visit will help you and your surgeon choose the optimal implant. Keep in mind that following the advice from a surgeon on this or any other website who proposes to tell you what to do without examining you, physically feeling the tissue, assessing your desired outcome, taking a full medical history, and discussing the pros and cons of each operative procedure would not be in your best interest. I would suggest your plastic surgeon be certified by the American Board of Plastic Surgery and ideally a member of the American Society for Aesthetic Plastic Surgery (ASAPS) that you trust and are comfortable with. Ultimately, in person consultation with well experienced board-certified plastic surgeons will be necessary. Careful measurements, dimensional planning, and careful communication will be key.Generally speaking, the best online advice I can give to ladies who are considering breast augmentation surgery ( regarding breast implant size/profile selection) is:1. Concentrate on choosing your plastic surgeon carefully. Concentrate on appropriate training, certification, and the ability of the plastic surgeon to achieve the results you're looking for. Ask to see lots of examples of his/her work. Given your concerns, I would suggest that you spend additional time with your plastic surgeon; ask the same question of him/her regarding the use of ultra-high-profile implants and the greater projection that results from the use of these breast implants.2. Again, have a full discussion and communication regarding your desired goals with your plastic surgeon. This communication will be critical in determining which operation and/or breast implant size/type/profile will most likely help achieve your goals.In my practice, the use of photographs of “goal” pictures (and breasts that are too big or too small) is very helpful. I have found that the use of words such as “natural” or "C cup" etc means different things to different people and therefore prove unhelpful. Answer: How do i choose what is best for me? Visit a cosmetic breast surgeon who is a BCPS. As a board certified plastic surgeon, we are trained in cosmetic and reconstructive breast surgery. Some that further specialize, like myself, get additional training specifically in cosmetic breast surgery. Familiar with the latest implants and techniques, I would recommend, measurements, sizing, and specifically sizing with a black and white camisoles...this will give you an excellent idea of the shape you desire for your body type.
