Hello I am 32 years old male I had a Vaser liposuction 6 months ago to shape my belly and abs. Only 2 litres have been removed. Unfortunately result is very dissapointing. Uneven areas, poorly done "sixpack" and skin damage. I don't want to continue with my current doctor for possible procedures and corrections. I wonder if there is a slight possibility of correction of the areas on the skin and irregularly, and what could be your suggestions? Your opinions are very valuable

Two liters of fat removal seems like a lot of fat from a patient with your abdominal anatomy. Nonetheless, let’s analyze your case. The abdominal anatomy consist of three areas: muscle, fat, and skin. 1) Your skin appears to be normal without any stretch marks, or any excessive loose hanging skin. Therefore, your skin doesn’t really need treatment. 2) Next is your fat. You do not appear to have much residual fat at all in any of your pictures. I believe this was treated with your prior VASER liposuction procedure. 3) Next is the muscle. This is where you have your major problems. Below your belly button, you have laxity or loose rectus abdominis muscle. Liposuction even in VASER liposuction will not correct muscle laxity. VASER liposuction only corrects fat. Therefore, my recommendation would be to undergo muscle plication to flatten the muscle from the pubic region to the belly button. If you look at carefully at your three-quarter picture, your rectus abdominis muscle even protrudes above your belly button. Therefore, I would plicate the rectus abdominis muscle from the pubic bone to the sternum. This will get you a flat abdomen on profile. If you want more ab etching, and more defined six pack abs, VASER hi-def six-pack ab etching can be performed at a later date to give further muscle definition. Unfortunately, you placed your body in the hands of a surgeon who is either a novice or a poorly trained surgeon. Having a VASER in the hands of a surgeon who does not understand anatomy and energy devices is dangerous and leads to this result. You almost got full thickness skin burns on your flanks with resultant discoloration that might take a very long time to resolve or will never return to normal skin. The VASER is a tool and if used inappropriately, damage can happen besides the poor results. The many liposuction skin puncture holes/scars are another indication that your surgeon is not trained in advanced sculpting techniques to deliver natural sculpted and "un-operated" results. As a Total Definer surgeon who trained with the inventor of the VASER Hi Def Lipo technique, I need to point out a few things about body sculpting in males. First, it is an all or none procedure. You cannot have a "six pack" and hanging boobs or gynecomastia. The chest, abdomen, back, waist, hips, lower back fat pad, pubic area need to be addressed together to achieve natural harmonious results. Second, one needs to place these small liposuction poke holes in areas that cannot be seen after surgery (low in the groin area, inside the belly button, within the areolas under the nipple, front and back armpits). Third, the surgeon has to be familiar with the underlying muscle and bony anatomy. You cannot "carve" fat willy nilly and call it a six pack. The liposuction has to be done both deep and superficial in the exact location where the inscriptions are, follow the anatomy. Trying to create fake symmetrical results is not natural. Fourth, your gynecomastia and chest sculpting was not done. A well sculpted abdomen (not yours) with men boobs is unnatural. The pectoralis major muscle and the latissimus dorsi muscles were not revealed. My advice would be to wait up to a year before seeking further surgery. You almost suffered major skin loss due to the destruction of the subdermal vascular plexus. Also, your future surgery is not a "correction". It is major surgery, difficult surgery and will be more expensive than your first surgery. Maybe the cost was low and the promise too high by your surgeon to entice you (?) Answer: Liposuction results To make an accurate assessment of a plastic surgery procedure we need to see proper before and after pictures. If you don’t have before and after pictures and ask your surgeon to forward, the ones they took. Early postoperative pictures are not helpful and without seeing a clear comparison of before, and after it’s hard to assess your outcome. I personally am not a big fan of high definition, Liposuction. It’s hard to see any obvious problems from your outcome and 2 L of fat. It’s probably a significant amount to have been removed. Based on the limited information you’ve included. Follow up with your provider or schedule a second opinion in person consultations with other plastic surgeons in your community. Liposuction is a permanent irreversible procedure. Improving Liposuction results. Once the procedure has been performed is extremely difficult. Revision work, or secondary liposuction is many times more difficult in the primary Liposuction. until you can clearly define what the problem is. There is no indication it to have further surgery. Revision work becomes more difficult, if the first procedure was done aggressively. Consider re-posting with quality before and after pictures and a clear description of what you’re hoping to change.
