I had a revision rhino/septoplasty 6 months ago to resolve two small, sunken divots in my anterior lateral cartilage that developed after a car crash. My surgeon placed diced septal cartilage grafts, but didn't do any other external work& no other areas were really swollen. However, I have significant swelling where one of the grafts was placed. It looks like a grain of rice was sewn under my skin. I’m worried the graft is too large. Is this normal? I have medium skin thickness.

Thank you for your question. In your case radix lowering and tip projection are possible. For more defined and projected nasal tip, using septal cartilage is preferred, but ear cartilage or rib cartilage can be used if septal artilage has been already used. Which one is more advantageous needs...
Improving nostril asymmetry is one of the hardest aspects of rhinoplasty. Yes you do have a bit of tip asymmetry which contributes to the difference in nostril shape. It may be that some scar tissue has developed in an asymmetrical way or that the tip was slightly asymmetrical after surgery....
Your issues can be addressed by lowering your radix and providing tip projection to fix your ‘polybeak’… a septal extension graft will take care of this….
