What would be the reason the surgeon would choose to make the cut this way (a wide triangle) rather than just horizontally?

I am not sure that I would recommend chin augmentation based only on these photos? However, in person consultation with 3D simulation and discussion about your goals, would help to make appropriate decision. Hope this helps.

You are a candidate for a genioplasty and your photo is very reasonable. Moving your chin forward and down would add length and projection to your chin, balancing out your profile. It seems that chin augmentation with implant would be appropriate procedure to address chin deficiency. In person consultation with a board certified plastic surgeon, possibly 3D software simulation, is recommended. Good luck.
