Wondering about recovery if I chose to move forward with a revision. Just a little bit of fat above the scar. Surgeon suggests a 6 inch cut and remove the fat. All in office with local anesthesia. Not lipo. Can I assume this will be nothing like the original TT? And what would recovery look like.

It is difficult to see exactly what is going on. This issue is due to either excess fat, or abdominal wall weakness (not necessarily a hernia)

It's difficult to assess your results from this limited photo. It would be helpful to see your before/after photos if you have them. If you are uncomfortable with your results you may want to seek a second opinion from another board certified plastic surgeon near you.
It’s not a good idea to partially tighten the muscle wall. Either do it stem to stern or leave it alone.
If you want to have a tight flat abdomen you need muscle tightening up to your rib cage.
