I got 415cc implants 2 years ago. At the time, th surgeon said a lift wasn’t necessary. He also mentioned that I could always do a fat transfer after the surgery if I ended up unhappy with the space between my breasts? I should have done more research at the time. I am now wondering what my best option would be to fix the gap so that I have cleavage, and to fix the asymmetry/drop. Do I need to switch both implants out and do a lift? I’m 100lbs, so a fat transfer seems tough.
Answer: Breasts Your implants are too large and heavy for your tissue to support. You need to go down in size and have your pockets tightened. You may need an internal mesh bra for support. Unless cadaver fat is used, a fat graft may not be an option for you.

Answer: Making things better Your breasts don't need a lift, your implants do: scar capsule tightening plus a resorbable mesh will achieve long lasting results. This will vastly improve dropout and subtly improve gap. Softening of the transition between implant and chest wall will be difficult without fat though. However, once the implants are in good position, with more muscle covering them, you will likely be happy.

Because your areolas point outward, it will be important to suture around the outer breast to keep the implants from shifting. This will also help obtain cleavage. You may need a mastopexy to address the “drop “ you were talking about. An incision would be made around your areola down the breast, and under the inframammary fold like an anchor.
