I have a really large nose with thick oily skin. My nose also spreads across my face when I smile which makes me really insecure. I also hate my side profile. I have been told that my type of noses are the hardest to operate on and results are not guaranteed. I dream of a small cute tip and a thinner nose bridge. I don't want a slight unnoticed different in fact I'm hoping to get a more dramatic change on the tip of my nose
Answer: Rhinoplasty Hello and thank you for your question. Based on your photograph, you have the appearance of thick nasal skin. You may benefit from a tip refining rhinoplasty. You may also benefit from an alar base reduction and a nasal tip deprojection. Make sure you specifically look at before and after pictures of real patients who have had this surgery performed by your surgeon and not just a computer animation system. The most important aspect is to find a surgeon you are comfortable with. I recommend that you seek consultation with a qualified board-certified plastic surgeon who can evaluate you in person. Best wishes and good luck. Richard G. Reish, M.D. Harvard-trained plastic surgeon

Answer: Nasal Tip The limiting factor in reducing the size of your nasal tip is the thickness of the skin over the nose. Improvements in the shape of your case can be made. A consultation with an experienced surgeon would help you understand what can be achieved for you.

Answer: Ethnic Rhinoplasty on a Thick Skinned Patient Dear maather29, While you do have thick skin and a wide nose rhinoplasty could provide very dramatic changes for you that would nicely balance and feminize your facial aesthetics. Personally I would suggest a open rhinoplasty so that you can reduce the nasal tip under direction vision. I would reduce the width of the bridge with osteotomies, refine the dorsal hump, and an alar base reduction to address your current concerns. I would suggest that you seek consultations with some experts in the field. You can have imaging performed to show you the proposed results taking into consideration your skin thickness and nasal anatomy. You can also review other before and after photographs of patients to ensure natural looking rhinoplasty results. See examples in my ethnic rhinoplasty link below and video below. Best regards, Michael V. Elam, M.D.

Answer: Middle Eastern Nose Yes you do have thicker skin, but this doesn't mean rhinoplasty can't improve things dramatically. For you I would remove a slight hump on the bridge and this would make the nose appear shorter from the front and more feminine from the side. It appears you do likely need your depressor septi nasi released, which is partially responsible for why your nose appears this way when you smile. Alar reduction will also help. Decreasing your tip size can also be achieved with open rhinoplasty. You are a beautiful woman and I think your goals can be achieved as long as you have a reasonable expectation and clear discussion with your surgeon. Hope this helps!
