For fat in the pubic region, there are two modalities that are nonsurgical that may help. These include SculpSure and cool sculpting. Although these modalities can reduce fat, liposuction tends to be more effective. Sometimes, lifting the skin in this area if one has had significant weight loss can make a significant difference also.
Since you have lost 136 pounds, more likely than not, your problem involves loose skin as well as excess fat. I do not believe your problem can be adequately corrected without surgery. Without photos, I can't be certain, but I suspect that you will need a combination of localized liposuction and excision of the redundant skin. Both these procedures can be easily done with local anesthesia in around an hour and can even be done without any sedation. Without sedation, you could drive yourself home from the doctor's office and for many types of occupations, you would not even have to miss any work.
If the fat is not shrinking as you loose weight, you may need liposuction. I would suggest meeting with a board certified plastic surgeon and discussing all your concerns.
Thanks for your question. This is not an uncommon problem. Liposuction is generally a good treatment for this site. It can safely remove the unwanted fat. Excess skin is sometimes a problem as well, and that would have to be surgically removed as a lift procedure. Best wishes.

Dear Los Angeles 4257,We surgeons hate the skin on the inner thighs! In most patients this skin is very thin and has terrible elasticity. The surgeries for inner thigh lifting are also not the greatest unless a scar can be run down the length of the inner thigh like the inseam of your pants....
You can use it either way. I would recommend using it in the manner that is most comfortable for you and what your surgeon recommends.
It appears to me that you have a recurrent diastasis or separation of your upper abdominal muscles. This may have occurred early in the post-operative process. You may also have a small amount of fat as well in that area that may be improved with liposuction - but your best bet is probably a...
