Please forgive me for the many pictures. I wanted to cover all sides. I’m a 23-year-old woman weighing 55kgs. I have 2 injection scars I got when I was very young and they resulted in indentations on both my butt cheeks which has caused me insecurities all my life. I have a lot of indentations and holes all over my bum and one side rises and the other butt cheek as low as seen in the pictures, I don't want a big butt but I would like to shape it. Kindly advise me. Thanks

Answer: Childhood injections and gluteal augmentation There are a few different things that need to be addressed in order to get you a better looking bottom. Most likely there is scar tissue that’s contracting and pulling in on the skin from the childhood injections. Adding volume without addressing the contracting scar tissue will leave sub optimal results. The amount of scar tissue may or may not be very extensive depending on multiple factors such as how many injections you had as a child. If there is only one small band of scar tissue pulling in the skin in the main area with the divot then releasing that scar tissue in conjunction with fat transfer should give it a good improvement though it may be challenging to develop a complete normal or ideal buttocks contour. Most likely the best result would come from a staged procedure. If fraying or separating the scar tissue becomes very involved or requires more surgery than anticipated then this will limit how much fat transfer can or should be done during the same procedure. Different providers will have different opinions on how to approach this but I think a combination of releasing the scar tissue and adding volume with fat grafting is going to be your best approach. Delivering consistent high-quality Liposuction and fat transfer results is more difficult than people think. For that reason it’s important that patients put in effort to find the most experienced and talented provider in their community. This is especially true for doing secondary work, revision work or working with unusual circumstances such as your own. Defined the most experienced and talented provider in your community I suggest scheduling multiple in person consultations with local board-certified providers who seem to have a knack for the BBL procedure. During the actual consultation I suggest you ask each provider to show their entire collection of before and after pictures especially the outcomes of unusual cases, revision of work or other patients who’ve had challenging situations. by asking the provider to show both good and bad results as well as their entire collection up before and after pictures you’ll get some idea of how much experience the provider actually has. Providers who show you the best three or four outcomes of their career are not giving a fair representation of what average results look like and certainly not an indication of what results will look like for someone who has a challenging situation. An experienced provider should have no difficulty showing 50 or more sets of before and after pictures. I also recommend reading all the reviews on various physician review websites and eliminating providers with any significant number of justified negative reviews.
