TRT does not actually remove fat cells, but might (not always) encourage what you have to shrink. Liposuction will actually remove a certain population of fat cells permanently.

Testosterone replacement therapy will not have an impact on your fat distribution.
For most men weight loss has the most profound effect on abdominal fat reduction since it will reduce both visceral and subcutaneous fat.
You may or may not be a good candidate for Liposuction.
Typically taking exogenous testosterone will trigger the pituitary gland to reduce natural hormone production and the dose will gradually need to be increased until you override the pituitary‘s influence.
I guess you’ll find out firsthand if the medication has an impact on fat distribution but I don’t think you’ll find that the drug alone as much effect on fat without doing something about eating behavior and exercise.
To get an assessment for Liposuction I suggest having multiple in person consultations with local board-certified plastic surgeons.
Delivering consistent quality liposuction results is more difficult than most people believe. The importance of choosing the most talented experience provider for this procedure cannot be overstated.
I don’t think plastic surgeons have a preference for doing liposuction pre-or post beginning TRT. I don’t think it will have an influence on the outcome of the procedure either.
In the end there are only two variables that matter when it comes to this procedure. The first is the candidacy of the patient and the second is the skill and experience of the provider.
Those are the only variables I suggest patients focus their attention to.
