What type of implants should I get to improve the size without a lift? (Photo)

Thank you for your question and photo. However, your photo is very limited to the nipple position. Breast augmentation is limited by your anatomy i.e., the width of the breast and distance between the breast and nipple position. Breast augmentation will give you the desired fullness you seek, but the location of the nipple may not be in a desirable position of the breast. Another consideration to keep in mind is the fact you mention you are prone to keloids. A lift will give you more incisions, which will leave scars unknowingly for keloids. This will need to be discussed with your plastic surgeon, fullness vs scarring. I hope this helps.

I am guessing that you posted a photo without full exposure because you did not want a host of surgeons telling you that you needed a lift and your are trying to avoid one. Hello, thank you for your question. It is hard to tell from the photo, I recommend you to schedule an email, Skype or in-person consultation with a board certified plastic surgeon.  Whether or not you will need a breast lift will be based upon how much the implant will inflate that envelope. 
A board certified plastic surgeon will be able to guide you to the correct size implant for your measurements and desires.  Measurements of the chest wall and breast imprint width as well as knowledge of all breast implant sizes and profiles should make this process easy. Try on a profile bra with various silicone moderate and HP sizers starting at the number of ccs your PS recommends under a tailored blouse, tight sweater, swimsuit, workout clothes, and other garments you typically wear so that you have the best idea of whether the garments fit as desired with the implant selected. Work your way up or down from that size to determine the size that boosts your self-esteem, and fits your anatomy.  Choosing an implant based on an aesthetic goal that exceeds your anatomic capacity may lead to complications and possibly revision.

Many women think that augmentation will solve their problem eliminating the need for a lift.  This is usually not the situation.  Approximately 30% of women who come to see me for an explantation wanted a lift initially but were converted by their plastic surgeon into having augmentation instead.  You fall into that category.  You need a lift, not augmentation.  The technique I recommend is The Bellesoma Method.  This will reshape your breast tissue creating upper pole fullness without implants, elevate them higher on the chest wall and more medial to increase your cleavage.  Vertical scars are avoided, nipple sensation (in 95%) and the ability to breast feed are maintained.  Later, fat transfers can be performed if additional volume is desired.  Implants are not lifetime devices.  8% fail within the first year and 30% fail within six years.  Fat transfers are permanent and incorporated into the body.Best Wishes,Gary Horndeski, M.D.

From what I can see from the picture, your nipples are pointing a bit lower than ideal but I have had patients who just wanted the volume without the lift or with the lift later. Either way, you need to decide what your priority is: larger breasts to look good in a bra until you really need a lift, or ideal result with nipples in higher position immediately and have more scars. Once you have had a pregnancy, or you get older, your breasts will change and probably sag. Then you can justify the scars for a lift. Best of luck and be safe!

Thanks for your questions. My answer to your question has to address several important points. I do have to include an important disclaimer: only an in person consultation can give you an adequate answer. First point is that for most patients, about one cup size increase means about 150 cc of volume. That varies with your height and weight but it's a starting point. So two cups means about 300 cc. Second point is that you seem to show some ptosis. That means that your nipple areolar complex is a bit low on your breast. Your best result would ideally mean a lift. But in light of your tendency for poor scarring, I understand your reluctance to put additional or any scars on your breast. Axillary (armpit) incisions could be a good answer. We seldom see substantial scars with axillary incisions, though they can occur.. The third point is that we want to position your implant at the level of your nipple. As implants grow in size, they get wider. If the middle of your breast implant sits too high, the nipple will appear lower. So that suggests a narrower implant. The down side is that could reduce your cleavage. A potential solution is to lower your inframammary fold, but your photo suggests a very well defined fold, so lowering it could cause a double bubble deformity. Putting all that together, one option is a high profile (narrow but tall) implant placed through an axillary incision. I need to add that a biplanar placement would be necessary to avoid moving the implants out too far and allow more cleavage. So how do I put all of this together and make it understandable? My personal preference is to use sizers to establish your volume desires and then use a three dimensional photographic simulation (I prefer Vectra) to show you what particular implants could do for you.To complicate matters, thought it is less precise and has some downsides, consider fat transfer. Again, only an in person consultation can allow any plastic surgeon to make definitive statements about your particular needs. Best of luck!
