Hi. There are very subtle differences between these implants. They are slightly different in their level of cohesiveness, and they come in different sizes. There are actually more cohesive implants that are available as well. Having said that, I use both of these for different reasons, and every person has slightly different requirements. Depending on your goals, as well as you breast and chest size, your surgeon can give you the pros and cons of each of these styles as they would relate to you. If you go to a surgeon who uses one or the other (but not both), you may not get as unbiased of an answer. Choosing an implant is very unique to each individual and is best done in person at your consultation when the surgeon can examine you and help you figure out what will give you the most rewarding outcome. Best of luck!
Dear paty1988,breast implant type and sizing selection is a complicated process and requires a detailed consultation that includes a discussion of your goals, a detailed examination, and a lengthy discussion. But the decision will always start and end with the patient. When it comes to sizing, we have all of our patients find 3 photos of their desired breast size. We ask for naked breast photos because clothing can obscure the true size (push up bras). Next, we consult with patients and go over the photos and take detailed breast measurements during a physical exam. Next, we using a sizing algorithm to determine a range of implants that will work based on the exam and the measurements.Lastly, during the operation, we use silicone sizers in the operating room to see how the size actually looks in the breast. We sit the patient up in the operating room, while asleep of course, and compare the sizer result with their desired look photos. If it is too small, we start the process over with a slightly larger implant. We don't stop until we get the perfect match.Of all of my breast augmentation patients, every one of them has been happy with their size due to this detailed and meticulous process. You don't want to invest time and money into a result that is either too big or too small. You should discuss your concerns with a board-certified plastic surgeon and make sure that your surgeon understands your goals.Daniel Barrett, MDCertified, American Board of Plastic SurgeryMember, American Society of Plastic SurgeryMember, American Society of Aesthetic Plastic Surgery
Hello and thanks for your questions. In all reality, there is very little, if any, appreciable difference between these 2 implant types. My best advice would be to find a surgeon that you trust and whose results you like - proceed with whichever implant they are the most comfortable using in order to get your best result.Best wishes!Daniel Brown, MD, FACS Board Certified, American Board of Plastic Surgery Member, American Society of Aesthetic Plastic Surgeons Member American Society of Plastic Surgeons Castle Connolly Top Doc Real Self Top 500
Hi and thanks for your question. Implant manufacturers today have a variety of silicone implants that vary in projection, texture and firmness. The firmness is a factor called cohesively and is graded by degree of hardness. This is largely determined by “senistoke” a rating of silicone liquidity. The companies have capitalized on this and the patients awareness of cohesivity and its relationship to visible “ripples”. While it is true that the more firm or cohesive the implant the less likelihood of ripple, there are other factors to consider when choosing implants. The higher the firmness the more upper pile and round the breast will appear, this may be good for you and others not. The more firm the breast implant the more firm you’re breast , sometimes this may be too much. The more firm the implant the Moreno it generally costs. Soft touch for example cost more than responsive and less than cohesive in the Allergan example. Also remember that the native tissue thickness a and the placement of the implant below or above the muscle and the hen technique of the surgeon are variables in appearance of ripples. The firmest of the Implants is Allergan cohesive, not soft touch. Use your judgement if the extra cost and firmness is really worth it. In my opinion the extra cost is often not worth the slight decrease risk. Discuss all the options with your surgeon. Good Luck hope that helps.
Both of the breast implants (highly cohesive gel with more "fill" per shell) you mentioned are excellent products when it comes to helping prevent breast implant rippling. However, there are many other "factors" that come into play; these factors are patient specific (patient body type, past surgical history, goals...) and surgeon specific (good judgment, precision of pocket dissection...).Personally, I think that careful selection of plastic surgeon and careful communication goals are the two most important "factors" when it comes to achieving the desired outcome with breast augmentation surgery. The specific breast implant manufacturer and even type of breast implant are far less important factors, in my opinion. My best advice: select your plastic surgeon carefully. Make sure that he/she has significant experience achieving the types of outcomes you will be pleased with. Then, communicate your goals carefully, as well. Working together you will come up with a good plan to achieve your goals. Best wishes.
