Thanks for the question. Certainly local provides adequate anaesthesia and if you are anxious an oral sedative hypnotic will take the edge off. We tend to inject local for its anaesthetic benefit intra operatively whether or not you are asleep. The advantage too, is a relative cost saving since you don't require an anaesthetist if local and oral sedation are used. There are less post operative anaesthetic complications as well, like nausea and vomiting which can cause excessive bleeding post op, also a potential benefit long term as well. Hope this helps.

Thanks for your question and photos. I would have to feel the ridges and perhaps order a CT. But if this is a frontal boss then really the only solution is to abrade them or reconstruct the frontal bone. I doubt that your muscle is the cause of this much elevation. So Botox shouldn’t work. If you fill the hollow your ridge will become even more pronounced. Seek a consultation with a craniofacial trained plastic surgeon for further details. Hope this helps.

Thanks for the question and photos.  You have a fairly good result but the only way to effectively flatten the remaining areas is with an erbium resurfacing laser. It will reduce the height in one session. The downtime will be significant requiring ointment for approximately 1 week. Make up will then be able to hide residual redness which may persist another 3-4 weeks. Contact a board certified plastic surgeon with experience in this technology and get further information. Good luck.

I had an accidental fall in 2013. Had surgery in 2015 to replace the philtrum with skin from behind my ear. There is still scarring and the one side had a bump that I can't see to get rid of. Do you suggest I try laser? I've also done the needling too.

Hi and thanks for your question. It is very satisfactory to use a smaller implant to enhance your existing breast tissue. 200 cc is maybe a tad small. Likely 250 would be better. To be sure see a board certified plastic surgeon in consultation. They will have you try implants over your breast in a bra to see what suits your needs precisely. Hope this helps.

I’m petite, 5’, 44 kg and have a slim/athletic, but feminine body. I currently wear a 32B/34A, have breast tissue and am considering implants with cc between 200 - 250. I have chosen silicone and round shape. I do not desire to have very large or obvious breasts. I am looking for a size that can give me similar proportions in the photos (not the same size) on my own body. I have attached a few photos of myself. The photo of me in the black bikini is to show my frame.

Thanks for your question.  I assume you are referring to the nubbin and that you have inversion of this or flattening of it. Proper surgical correction using a release under the nubbin and turn under areolar flaps are far more likely to persist and scar minimally. Filler like Radiesse has sufficient lift if the nubbin is at or above the surface but only lasts 18 months. A cartilage graft would be a better solution in this case. Hope this helps.

Shoulder depressions and problems finding a proper fitting bra are also stated. Realize that the procedure when done expertly will bring you great satisfaction. Therefore, be sure to consult with a board certified plastic surgeon and get a detailed assessment of your breasts and a proposed approach to a safe and favorable outcome.

Thanks for your question and photos. The situation isn’t too bad. The mastopexy would need to incorporate GalaFlex to maintain a higher position and an extra high profile implant would help too. The quality of the scars isn’t the best. So next time wear silicone nipple scar shields or tape to improve them. Hope this helps.

I got a Mommy Makeover with a breast lift &amp; 195 cc gummy bear high profile implants 1.5 year ago. My natural breast tissue was already a C or D cup so the Dr. chose a small implant. On one hand I am happy they look natural. But in a few years, I would like to go in for surgery again, &amp; this time I'm hoping they will look more fake I guess. When I go back, how do I achieve a more large, round, perky look? Especially when laying down (for my husband). Bigger implants? Another breast lift?

Thanks for your question. Assuming that the average nipple areola is 45 mm in diameter then the amount of skin removed will be larger than this by 20 mm or so in order to correct the skin redundancy. If the the opening is situated in a non-concentric fashion some centralization would be possible as well. However, it would be necessary for you to be examined by a board certified plastic surgeon to ensure that you are a good candidate for this, and that you will be satisfied. I’m interested in a donut lift and I’m wondering how much skin, on average, can be removed while minimizing puckering. Also, my areola are larger than I’d like them to be. I’m assuming that because concentric circles must be made to remove excess skin, resizing of the areola is built in to the procedure, right? Finally, can a donut lift reposition the areola/nipple so they’re higher up on the breast?

Very good question. Even experienced injectors can cause this complication. However, signs include painful swelling and discoloration of the skin an necrosis in the lateral corner usually due to occlusion of the labial artery. Treatment starts with injection of an enzyme to breakdown the hyaluronic acid in the area. Implant injection needs to be limited in this location and  slow injection with knowledge of the artery location is critical to avoid this. If concerned that this has occurred immediate action is required. Contact a board certified plastic surgeon as soon as possible. Hope this helps.
