Parts of Areola missing after breast lift/reduction and implants. What happened and what can be done? (Photos)

I was curious to find out any suggestions or ideas why my right areola is now distorted. I originally had 36 DDs that were grade 3 ptosis. My surgeon suggested a reduction and implants to raise my breasts with cleavage. I went to every follow up appointment but only saw my surgeon once since the surgery. Her PA was in charge removing my sutures, did she not remove them soon enough? The PA suggested revision soon after as my scars were not great looking, now she suggested a tattoo.
