The best way to answer your question is to ask yourself what kind of results you wish to have from a procedure. Thermitight, Ultherapy, and other less invasive technologies have the advantage of avoiding a surgical recovery, but have the disadvantage of a much lesser result. In my experience, most patients with the aging changes shown want a fairly substantial result, and this means having a facelift with platysmaplasty. Some patients are willing to have a lesser result because they are unwilling or unable to have surgery. However, for other patients, these lesser results end in disillusionment despite having been counseled about expectations. The biology and physiology of aging haven't changed and there is no minimally invasive technology that produces similar results to surgical intervention.
The aging neck is often characterized by two distinct processes: loose skin and excess fatty tissue. The ThermiTight procedure targets the loose skin component. As a plastic surgeon, I would say that your most dramatic outcome would be with surgery. However, I also understand that not everyone is willing to undergo surgery. As far as non-surgical options, I would recommend the new Profound radiofrequency microneedling for the lower face/neck area. It is the only device that allows for both collagen and elastin to be formed in response to the treatment. The results are not quite a replacement for surgery, but it can help reduce some of the laxity in those areas.
