I hate my side profile. My face is too flat, chin too long, nose too big, and tiny downturned lips. What would I need to do to fix this?

Answer: Rhinoplasty Hello and thank you for your question. Based on your photograph, you may benefit from a tip refining rhinoplasty. Your surgeon can accomplish this by trimming, suturing, and reshaping the cartilage in your lower nose. You may also benefit from a conservative dorsal hump reduction. Chin reduction surgery can be performed at the same time. Make sure you specifically look at before and after pictures of real patients who have had this surgery performed by your surgeon and not just a computer animation system. The most important aspect is to find a surgeon you are comfortable with. I recommend that you seek consultation with a qualified board-certified plastic surgeon who can evaluate you in person. Cheek implants are placed for patients in the mid-face which allows augmentation of the cheeks. A full set of facial photographs are required to make any determination about two different surgical procedures that are both three-dimensional operations.

Answer: Rhinoplasty, injectable filler to the cheeks and injectable filler to the lips Thank you for posting your question and photo. It would be helpful to have higher resolution photos from the front, both sides, below the nose (base view), and angled (about 45 degrees) from both sides. Based on your photo, you have excellent profile to the jawline, chin, and neck. Be happy that you have a strong chin, as this is a feature many people wish to have. Your cheeks have somewhat less prominence and this is the reason that your chin appears to be overly long. The addition of filler to the cheeks (I prefer Voluma or Restylane Lyft) will add volume and will balance the midface (cheeks) with the lower face (jaw and chin). In addition, you may benefit from filler around the eyes to improve the contour from the lower eyelid into the cheek. My preferred filler for this area is Restylane-L. Your lips can be enhanced with the use of filler to in and around the lips. There are many good options for this including Restylane-L, Restylane Silk, Juvederm Volbella, and Juvederm Ultra XC. With regard to your nose, it is difficult to determine how best to address your concerns without seeing the additional photos I mentioned. Overall I do not feel that your nose appears to big for your face. You may be a good candidate for rhinoplasty to refine the tip and potentially to reduce the very slight dorsal hump (bump) on the top of your nose. I recommend that you consult with a board certified facial plastic surgeon or plastic surgeon who is skilled in facial volume enhancement and rhinoplasty. This will allow the surgeon to take standardized photographs so that both you and the surgeon are on the same page in understanding your desires. It also allows image simulations of the nose so that the surgeon can demonstrate potential changes during rhinoplasty and give you an understanding of the results that can be achieved. Best of luck!
