It’s not possible to inject local anesthetic into bone.

Soft tissues a relatively easy to anesthetize with local anesthesia.

Dentists rely on something called nerve blocks primarily to get patients comfortable for dental procedures.

If a nerve block does not completely cover the area being worked on because of cross innervation of different nerves that may be difficult to reach in the area could be painful.

Personally I would do this with general anesthesia or deep sedation regardless with an anesthesiologist.

Oral surgeons typically have better training and more experience in doing intraoral nerve blocks.

Some plastic surgeons may have experience and feel comfortable doing this as well.

Each provider will have their own view and you’ll simply need to have consultations and find out what each one has to say.

It doesn’t matter if someone across the country is willing to do it using only local anesthesia if the providers in your community are not.

It’s always a good idea to have more than one consultation so I suggest scheduling a few of them both with oral surgeons and board-certified plastic surgeons who have experience doing this kind of work.

Price is no guarantee for quality work. That said, the most talented and experienced providers usually don’t offer the lowest prices either.

My recommendation is to focus on finding the most talented and experience provider then follow their recommendations.

The most expensive cosmetic surgical procedure can be the one that’s poorly done.
