Which procedure would be more suitable to lift a saggy bottomed out breast with old implant with ripples due to the lack of fat tissues (skinny patient)bottom out. The Internal bra or bellesoma? Of course I’m replacing the implants but doesn’t want to do fat transfer. Breast size is c ( It’s 20 years old so I’m not sure) weight is 119 with some fat in the hips area that surgeons I consulted with said it can be transferred to fix the ripples but I hope I don’t need to do that.

You appear to have a capsule. You can try oral Accolate and external ultrasound treatments to see if the capsule can be softened. Otherwise, you will need surgery.
You need a lift. Please do not go larger for you will just have larger and more sagging breasts. The augmentation did not correct the need for the lift.
You need a lift. Do not go larger. What you have now it too large and heavy for your frame and tissue. if you go larger, you will just sag even more. An internal bra to give support could be helpful.
