Removing a callus/bony bump from the bridge of the nose.

Question

I had a full septo rhinoplasty on march 2015...i then had calluses rasped down on feb 2016. But the bump has reappeared. My surgeon seems to think its still calluses. If this was to be readdressed what could a surgeon do to ensure calluses doesnt form again.

Hello, Thanks for your question. If you need a revision rasping of this same area, it may help to add some low dose steroid wash into the area to slow down some of the healing. The hope is that reduced inflammation from using the steroids, reduces the drive to lay down more callous. Speak with your surgeon about this. Additionally, serial taping of the bridge and manual massage may help. Good luck, Dr. Shah

rhinoplasty revision for residual bump? It is unlikely that this will recur with another filing. Your surgeon did not want you to have a dip where the bump was and that is why he filed down the bump as he did. Im sure he will be more aggressive since you form scar more than the average.

Mild  bumps or irregularities may be perceived after he rhinoplasty especially if you have thin skin.   However  the whole bump should not reappear.  When that happens my concern is that  possibly not enough was removed in the first place.  My recommendation is to seek another consultation by an experienced rhinoplasty surgeon.  If the previous procedures were done using a closed technique then an open technique should be used to completely  visualize the anatomy and addressed problem directly.  If you have thin skin the bone and cartilage should be covered with a blanket of temporal fascia that will help smooth the contours and minimize contour deformities that you can see or feel.

Schedule a follow-up appointment with your surgeon.  Occasionally a callus can return, and it would take a short revisionary surgery to rasp it down again.   In addition, you may benefit by taping your nose postoperatively.  Good luck.

It's important to know that bone and cartilage is living tissue, and not static, and calluses can grow as a result of surgical manipulation.  The majority of  patients do not get a bony calluses of the nose, but it is common problem  that can occur approximately 10% of the time. The simple rasping of the bridge line by the original surgeon can address the issue under a brief general anesthesia.

In my practice, I have had one patient that required re-rasping to remove a stubborn bump; you just need good hemostasis so that blood products don't accumulate back and form a stubborn hump; I perform any re-rasping of patients under local to save them extraneous fees associated with surgery center and anesthesia.

It should be removed by a revision expert.  This can usually be done in the office.  For a recurrent callous or lump, a chisel would be your best bet.  Rasping the boney lining (periosteum) can stimulate callouses in some patients.  Best wishes.

I suggest you have your surgeon rasp the small bump a second time. Despite your previous history you are not at greater risk for this to happen again.
