Im 27 and had breast reconstruction following a double mastectomy in 2014. My PS went with a teardrop 425 implant that gave me a very natural but small look along with a lack of cleavage or ability to create cleavage. Over the past few years I've been called flat chested and am hoping to obtain a bit more volume and cleavage while maintaining the natural shape. I considered switching to a round implant but am afraid I'd lose the natural look. I'm 5'4 and 125 lbs and would love to love my body again.

Answer: Fat grafting use in breast reconstruction Hello and thanks for posting your question!Fat grafting us used widely by surgeons to augment breast reconstruction. Fat grafting is borrowing fatty tissue from one area and transferring it to another area, where 60-70% of that fat is expected to survive and thicken the tissues to where it's been transferred, thus improving the overall contour of the area. The expectation of need for multiple rounds of fat grafting must be set if large volume increases are desired. With multiple rounds of fat grafting and tissues that will accomodate additional volume, significant improvements in the shape and volume of breast reconstructions can be achieved. Sometimes use of tissue expanders is necessary to increased available space. Each surgeon may approach this slightly differently. It would be worth discussing your concerns with your plastic surgeon and options for achieving a more acceptable result.Best wishes in achieving your goals.

Answer: Options You have several options. Fat grafting will certainly help the upper pole fullness but only to a certain degree. The anatomical implants are textured so they move less. Wearing a bra with padding on the outside won't be able to push them together and create cleavage. Smooth round implants are often the best choice for a breast that "moves" like a natural breast. Discuss it with your PS and see what they think.
