Hi thanks for your question and pictures. It seems like you have done well after removal of the cyst and not had any recurrent cysts or sinus tracts form. It is safe to have a BBL performed. No fat is injected into the area where the pilonidal cyst was. Often incisions are placed along that central fold as access points for fat injection. Depending on your previous incision and exam it may be ok to use that as an access point. This will be determined by your surgeon. He/She may opt to perform liposuction around the sacral diamond area (lower central back). It is ok to perform liposuction in this area as long as physical exam does not reveal any irritation or sinus tracts related to the previous cyst.
An in person consultation would be best for a detailed exam and evaluation, always consult with a board certified PS best of luck.
Dear fancy31788,If you are considering a surgery, I would suggest you to consult a board-certified plastic surgeon. Only after a thorough examination you will get more information and recommendations.Daniel Barrett, MD, MHA, MS Certified, American Board of Plastic Surgery Member, Am. Society of Plastic Surgery
