Hollow superior sulcus can be fixed with fat grafting or filler. Asymmetric appearance of the lids can also be due to ptosis of one eyelid. When one eyelid is slightly lower than the other, you often get raising of the brow. This creates a deeper appearance of the superior sulcus. Best to see an eyelid expert for an in person consultation.

This can be improved with fat grafting to the upper brow to reduce the deep sulcus. Also, an upper lid bleph that corrects a slight congenital dehiscence of the levator from the tarsal plate will help place the upper lid sulcus fold closer to the lash line as in the opposite side. This would be done with a bilateral upper lid bleph, only removing skin on the unaffected side, and correcting the dehiscence on just one side.
