i'm looking for a breast revision, i had silicone implants,but i think after 5 years are shaggier and the size doesn't please me.I want to go bigger, but round, kinda perky and i don't care for the natural look.I read about internal bra and the gummy bears,but they're anatomical shaped. I don't care about the cost I just want the result, not that i have now.. thank you

These implants have "cohesive, cross-linked silicone" bonds, that may prevent leaking of liquid silicone after rupture, and help to maintain the shape of the implant. Despite the release of a newer version of the silicone implant--one that has an anatomic "teardrop" shape--the cohesive implant is not new. Previous iterations of round cohesive implants have been available since 2006. The current "gummy bear" silicone implant is the 5th generation of silicone implants on the market. The result is an implant which hold it's shape in the upper pole giving a more round and full upper pole. I have done a lot of these new implants and with very positive feedback from my patients. The majority of these patients have consulted with me for revisions because they were wanting more upper pole fullness.

Thank you for the question and goal picture. Good advice would necessitate viewing pictures of your current breasts or in-person examination. To answer your question, cohesive silicone gel breast implants come in round as well as “anatomical” shapes. Keep in mind that, sometimes, patients looking for the full "round, fake” look benefit from saline breast implants. Of course, there are pros/cons associated with each type of breast implant option.
Best to seek in-person consultation with board-certified plastic surgeons who can demonstrate significant experience achieving the results you are looking for. Ask to see lots of examples of the work and communicate your goals clearly.
In my practice, I find the use of goal pictures, such as you have posted, very helpful during the communication process. Sometimes, depending on how much negative breast tissue of patient has, this "round” look is not achievable without additional maneuvers being performed. Achieving realistic expectations is part of the preoperative communication process.

If the posted photo is your goal, gummy bear implants are not right for you. The 410 gummy bear implants are anatomic (shaped) and will nog give your the look that you want. Most likely, you will need ultra-high profile silicone implants in subglandular plane. However, you have to understand that both natural breast tissue and implants will sag with time. Larger breast/larger implants will sag faster and have higher complication rate.

Regrettably the characteristics of large saline implants (>400cc) include displacement, capsular contracture, stretching of the skin brassiere, which eventually necessitates mastopexy. Revision sometimes will include the use of ADM (acellular dermal matrices: Strattice, etc.) as an internal bra, ultrahigh profile implants (saline or silicone) +/- mastopexy. Allergan Style 410, Mentor CPG and Sientra "gummy bear" gel implants are all "tear drop" shape, which will not give you the desired upper pole fullness. If this is a priority you may consider the textured high profile round implants. Hope this is helpful.
