Hi there, I would like some advice on whether I need a graft for primary rhinoplasty. I have surgery booked for January and my surgeon has said it's not necessary but I'm worried it may influence the final result. I was also considering going on a course of accutaine before surgery and continuing through the healing journey but the surgeon said this also is not necessary. I'd really appreciate your professional opinion. TIA

Answer: Grafts for thick skin Grafts are not necessary for the rhinoplasty unless we are trying to augment it like big change or very high tip etc. The cartilage that is removed from the tip will help and can be also used for any grafts that might be needed. Accutane should be discussed with your dermatologist before you decide. Regards Dr. Janjua

Answer: Rhinoplasty Hello and thank you for your question. Based on your photographs, you are a great candidate for rhinoplasty. Maneuvers for your surgery would include dorsal hump reduction, tip refinement, nasal tip deprojection, tip elevation, among other maneuvers. Cartilage grafts are extremely important for structural support and a good longterm result. With good surgical technique, you could have an outstanding result. I recommend that you seek consultation with a qualified board-certified rhinoplasty surgeon who can evaluate you in person. Best wishes and good luck. Richard G.
