Ethnic Rhinoplasty - Possible Pinched Nose or Something Else? (Photos)

Afroamerican rhinoplasties are of very high complexity and have very well-known and specific technical requiremetns, encompassing long surgical procedures that are exhausting and not made for every surgeon.

Side views are needed but, to start with, seems your absend (due to ethnicity) bridge was lowere further down when instead should have been raised or left untouched. You are right, you have alar collapse and still a boxy tip lacking projection and definition because the surgeon did not applie the suitable graftings and neo scaffolding to reshape a new nasal structure, the ensuing notch is a sigh of weakened alar support.

Seek better opinions from surgeons reliable on revision and ethnic rhinoplasties; see below the link what I mean.

The indentations can occur as the nasal skin tightens. At 1 1/2 years out it should be pretty close to a final result. Before considering revision surgery, filler can blunt those creases so it looks more like an earlier post op photo. If the filler works well, then silicone can be used to give a permanent effect. Be sure to see a rhinoplasty expert for injection.

When the supra_alar crease leads to a line that extends down to the nostril margin, this can be stigmata of previous nasal surgery. In our experience, conditions like this are well addressed with permanent non-surgical nose job treatments to fill in these lines, to achieve a less operated nasal tip appearance.

Hello and thank you for your question. Based on your photographs, you may benefit from a revision rhinoplasty with placement of mastoid fascia tip graft to help smooth out the tip and treat the soft triangle notching. With good surgical technique, you could have an outstanding result. I recommend that you seek consultation with a qualified board-certified rhinoplasty surgeon who can evaluate you in person.

It is unlikely that the pinching will get worst. The nose does not have a significant pinching. It is not noticeable to the observer. I would leave it alone for now. If you want to change it at some point it can be corrected to some degree with grafts etc.

It does not look like pinched tip deformity. It is result of your cartilage/soft tissue distribution; if that bothers you a lot you can discuss application of filler. It may be sufficient to correct this mild indentation. Good luck.
