You have to be careful with buccal fat reduction in male patients. It should be very conservative else it can feminize the face or make it hollow
From your pictures it appears that you have skin laxity and for that same reason I wouldn't advise you to move forward with the procedure simply because with the buccal fat pad removal you're taking away volume from the area so your face will look more contoured but since the quality of your skin doesn't appear to be ideal your results will most likely be over shadowed by the laxity in the area becoming more notorious.
The risk is very low but patients while on Accutane can develop more aggressive scar tissue. Being that buccal fat pad removal is an elective procedure I highly suggest that you wait at least 6 months after you complete your treatment and that way you can eliminate that risk of an unwanted result.
