I had fat grafting on my chin, but I feel like it still doesn't look good. I have also had orthodontic treatment and currently only wear retainers and my teeth are straight. Also, I don't like the way my lips point down a little diagonally. Finally, I would like to know if it is possible to perform it with local anesthesia.

You could definitely achieve this outcome with a genioplasty. Potentially add some chin/jawline contouring for an even more defined shape. One of the benefits of a genioplasty is that it will add length and definition to your jawline regardless!
