Question

I️ grind my teeth every night and I know this is for plastic surgery but is there any surgeries that could replicate the effects of the second pic? In the second pic I have on a mouth guard that increases vertical length.

It looks as if you might have a slightly diminished jaw line which makes your face appear shorter. I would consider using Voluma, an injectable filler first, that can last several years. This would make sure you like the effect and it is also reversible. If you like the effect, then you would be a candidate for an anatomic chin implant which wraps around your lower jaw to improve your angle and facial length and projection. I would recommend seeing a facial plastic surgeon in your area.

Depending on your examination, chin implant surgery may be considered for adding vertical height to your chin. Since our lower jaws are at a 45 degree angle to the floor, adding chin projection tends to increase vertical height. Hope this helps! Dr Joseph

What you are demonstrating is a vertical lengthening of the lower jaw which will increase your overall facial height. This can only be accomplished by a wrap around jawline implant that id design to mainly lower the inferior border of the lower jaw.

Hi lineyl,Based from your photos, you seem to have a weak chin and prominent jaw angle. While chin implant will help you get a longer face, we highly recommend considering jaw angle reduction as well or at least botox for the masseter muscle to further improve your facial harmony.

Hi, I have performed many facial shaping procedures, including Chin Augmentation with dermal fillers or silastic chin implants, for over 30 years.  From the photos, the lower third of your face is equal to the mid and upper thirds indicating that vertical length is not the issue but rather that the chin itself is quite weak.  When the chin is weak, this creates an imbalance making the nose appear larger, the mid face top heavy, the lower face looks short, de-emphasizes the lips and allows early formation of a "double chin".  
 Proper placement of a silastic chin implant adds forward projection to the chin thereby creating harmony and balance to the lower face.  Using the same incision, liposuction can be performed to reduce the fat and further shape the neck.  Excess skin, from below the chin, can also be removed through the same incision.  
 I have found that placement of a silastic chin implant, through a small curved incision under the chin (also allows excess skin removal) to be very safe, quick, highly effective and far less invasive than a sliding genioplasty (requires extensive tissue dissection, bone cuts and placement of metal screws and plates to secure the cut segments of bone).  I perform chin implant surgery in 30 minutes or less, often using a local anesthetic alone.  In my opinion, you are a good candidate for chin implant surgery.
