Explant better through the areola or inframammary fold?

Question

I had my first implants through the areola and removed last year through the same incision. They turned out deformed. The muscle flex deformity i guess... see my pics. They healed a bit but never looks normal. I lost the lower breast pole. Now I have had implants for 5 days and getting them back out next week. I hate them. Should getting them removed through the inframammary fold help prevent the flex deformity? or what can i do to prevent?

If you are having both implant and capsule removed, you will need to have it done through the inframammary incision. Through this incision I would also have access to your muscle, which I would repair to minimize potential deformity. Please see my video.

Personally I would rather remove the implants through an inframammary incision although this could leave a visible scar and it is possible to remove through the areola scar. I would personally tend to leave the implants in a little longer to be able to truly assess the result and you should not rush this decision.

Based on the picture posted you have a very difficult set of problems and anatomy.There is no defined infra-mammary line, a probable double bubble, wide sternum, low lying breast on the chest, and a very small breast. At least two surgeries and a third coming up.In cases like yours, in my practice, I want to see pictures before the first surgery, and after and before the second surgery. I want to see both operative notes. Extensive consult and examination in person, thorough analysis and discussion with you of all the possible options.The breast animation is due to the action and new attachment of the muscle on the breast tissue.This needs to be corrected and re-attach the muscle back to chest wall.The final results are impossible to predict, and may require more than one surgery to correct and achieve reasonable results.
