You will have the best results from a combination of treatments.  Fillers can give support to the underlying tissues and soften the appearance of the lines by the side of your mouth. Downturned corners (commissures) of the mouth can contribute an aged look and the development of marionette lines.  Chemodenervation (Botox) of a small depressor muscle at the corner of the mouth and volume filler of the marionette line area can improve the "droopy" appearance significantly.  Good luck!

You have a couple options depending on how far you are willing to take things.
If you're looking for a quick, temporary option, then selective neurotoxin (e.g., Botox) to the depressor muscles as well as filler/fat/PRF could help to improve those areas.

If you're looking for a more "permanent" solution, then a facelift is warranted.
