Question

I am 52, married, and in excellent health. I am considering BA to have bigger breasts and more upper pole fullness. I did breastfeed 2 children. I love my nipple position and generally love my breasts. I also am naked, a lot, in public when we travel abroad. I do not want a breast lift. That said, I feel like that the standard response due to my age and the fact I am not flat-chested is I must have a lift. Sharing 1 Wish Pic as last photo.

breast implant type and sizing selection is a complicated process and requires a detailed consultation that includes a discussion of your goals, a detailed examination, and a lengthy discussion. But the decision will always start and end with the patient. When it comes to sizing, we have all of our patients find 3 photos of their desired breast size. We ask for naked breast photos because clothing can obscure the true size (push up bras). Next, we consult with patients and go over the photos and take detailed breast measurements during a physical exam. Next, we using a sizing algorithm to determine a range of implants that will work based on the exam and the measurements.Lastly, during the operation, we use silicone sizers in the operating room to see how the size actually looks in the breast. We sit the patient up in the operating room, while asleep of course, and compare the sizer result with their desired look photos. If it is too small, we start the process over with a slightly larger implant. We don't stop until we get the perfect match.Of all of my breast augmentation patients, every one of them has been happy with their size due to this detailed and meticulous process. You don't want to invest time and money into a result that is either too big or too small.You should discuss your concerns with a board-certified plastic surgeon and make sure that your surgeon understands your goals.

Based on the pictures, it looks like you have a pseudoptosis (the nipple is above the fold). It is caused by deflation of the breast and not drooping.

Breast implants could restore fullness by adding volume.

My breasts are slightly saggy, but they don't necessarily need a lift. Creating a dual plane pocket through inframammary fold incision and using a high projection implant is advantageous for improving the sagging shape. If you use large implants like the desired picture, the sagging may become worse, so for the better shape is beneficial to use not too big implants.

Thanks for your question! Based on your photos and the position of your nipple-areola complex, I think a lift will be your choice understanding that your breasts may hang a bit more natural than a younger, less ptotic patient. I would suggest a bit of a fuller implant to kick that nipple up a little bit. Good luck!

Based on your pictures, you may benefit from having breast implants combined with a circumareolar lift combined with breast implants. While you may benefit from having a more invasive breast lift, you have stated that you are happy with your nipple positioning and thus, this type of procedure that includes a "doughnut-shaped" incision around the areola to remove tissue and the skin may give you the outcome you are looking for.

Thank you for your question and photos. An examination in person would be required to properly evaluate you and make specific recommendations, however is appears you would be a good candidate for a subfascial breast augmentation probably with moderate profile (classic or low projection) implants that would fill out your breasts naturally and tend to help avoid a “waterfall” deformity long term. (Please be aware that you may still be holding a pencil beneath your breasts without a breast lift.)

Yes, you can have implant to have larger and fuller breasts and more upper pole fullness, especially if placed behind the muscle. You do have some mild sagging now and your breasts naturally sit somewhat low on your chest so you will not have high perky breasts.
