I think your questions are among the hundreds that patients having elective surgery abroad need to address BEFORE they have surgery. I certainly don't mean to pick on you but, without a doctor in the states, many of these ancillary issues are left for you to handle without assistance. You could try talking to your Internist or Family doc to see if they can help you.
Disability forms should be filled out by the treating physician. You will have to ask your employer and the state if they will honor a form filled out by a doc not licensed in California. If not, check out in advance if your primary doc is willing to help. All the best.
Disability forms are filled out by the surgeon, so have your Mexican doctor handle that. As to whether your company would follow his directions, you need to take that up with the company. Make sure you have a good emergency fund set aside because any com-plication like blood clots, post op bleeding and or infection will require an emergency trip back to Mexico, and these things are just a recognized risk to surgery that you pa your surgeon to handle.
