I'd like to correct the mid face in conjunction with the nasolabial folds. Also improving the deflation/wrinkling around the eyes crows feet when I smile. I'd like to know the procedure recommendations and photos of similar patients that had the same concerns. Seeing is believing. TIA.

Answer: Fat transfer or a lift for midface deflation You Woukd benefit from a deep plane midface lift and fat grafting both. You have significant laxity not only if the skin but the SMAS and for a natural long lasting g appearance that must be lifted as well as the skin. Fat grafting g for the deflation with help but by itself has limitations and in large Amounts can change facial shape in an unnatural way. The nasilabial folds are so deep that they may benefit from additional filler or fat grafting and can be improved but not corrected. In extreme cases direct excision has been performed but I Woukd not recommend that.

Answer: Botox and fillers Very likely that Botox should be the first choice. Filling and volume augmentation of the cheeks would improve your midface. However, fillers for heavy nasolabial folds would not make much of a change; direct excision (with scar) may be your best solution for that problem. In person consultation or teleconference with a board certified plastic surgeon with excellent reputation in facial rejuvenation is recommended. Hope this helps.

Answer: Lots of options! Hi sir! You have lots of options going forward, depending on how aggressive you want to be, what bothers you the most, and what your budget is. For the midface/nasolabial folds, a large portion of this is due to midface volume loss (bone loss) and also your regular fat now falling downward (unfortunate aspect of aging). Filler or fat grafting are options to add back some of the volume in the midface and will definitely improve how this appears. However, it will not address the issue of the fat that has fallen -- only a surgical lift can do this. You want to be cautious with adding back too much volume without doing a lift as well because then it can make you look overfull. If you aren't ready to get a facelift, then I would definitely consider filller vs. fat grafting but be mindful that your surgeon does not "overdo" you. For the eyes, getting on a regular regimen of neurotoxin is key to minimize the crow's feet. This will take a while (years!) to unwind but it will definitely improve this area by preventing your eyes from crinkling when you smile. To address the lines that are already there, I strongly recommend a very good skincare regimen (consult with derm or aesthetician) because adding back moisture will plump up the skin to help it bounce back from lines. Additionally, laser is an incredibly powerful tool to help address fine lines such as those around the eyes, although you may require multiple sessions. If you consulted with me and had an unlimited budget and were healthy and otherwise a good candidate, my suggestions would be: browlift, upper bleph, fat grafting, face/neck lift, fractionated CO2 laser upfront, with regular maintenance of neurotox and good skincare.
