High forehead with recessed corners, looking for more feminine shape and lower hairline. What are my options? (Photo)

You first need to have an exam.  More specifically, you need to make sure you do not have androgenic alopecia.  Feminine hair line can be achieved with hair transplant surgery, lowering the hair line and rounding out the face (corners) for a more feminine frame to your face.

Hairline advancement versus hair transplants for lowering hairline in creating feminine shape.These are your two options. Hairline advancement will give you a natural looking hairline and a more feminine appearance can be created at much less expense than hair transplants. You do not have to wait for the hair to grow as with transplants one year. Also the hair texture is not changed with hairline advancement as it is with grafting. Whether you choose a hairline lowering through transplants or a trichophytic incision and scalp advancement technique as your initial procedure to lower the hairline, you will likely require further treatment in the future if male pattern baldness begins.  You should of course use medical therapy such as PRP, Rogaine, etc..., but if you loose hair along the hairline incision, this could be disguised in the future with transplants.  If you go the hair transplant route initially through FUT or FUE, then as your hair loss progresses in the future, you would likely undergo additional grafting procedures behind your hairline.  You did not provide a complete picture so it is impossible to tell whether you are male or female, but this answer pertains to males.  

For women, there are two ways to accomplish this, the first being a hairline lowering surgery where the hairline is advanced forward. It is often limited in the distance you can move the hairline, often around 1 inch or so. If there is more movement required, a hair transplant lowering procedure works well also. In some women, there is loss of the corner hairline and/or the temple peaks and when this happens, only a hair transplant lowering procedure can address these corners.

If you are genetically female, a one or two stage surgical hairline lowering process would work best for you. A genetic male would have to take in consideration that male pattern hair loss might affect the long term result. 

This question is frequently asked by patients who inherited a high hairline or it was elevated secondary to a previous cosmetic procedure. I am biased but I strongly recommend the Irregular Trichophytic Hairline Incision technique that I described many years ago. With this approach the frontal scalp is lowered so the patient has hair with normal uniform thickness and density behind  their hairline. Hair grows through and in front of this incision giving the patient total freedom of styling.
