I have a large dent in the top of my thigh. I believe it is caused from repeated bruising by my german shepard jumping on me and landing in the same spot. I remember a large deep bruise there before the dent appeared. The skin is still soft but is pale in comparison and the hair follicles enlarged. What can I do to repair this area, if repair is an option? Fat injections? Surgery? Thank you Cheri

Fat grafting would be a good option to fill the dent.  If there is scar tissue that is tethering the dent, that would benefit from release as well.  For deeper dents like yours, it is likely that more than one round of fat grafting will be needed.Best of luck!

A hematoma from previous trauma could cause this type of deformity.  A steroid shot that wasn't deep enough could as well.Fat grafting would be a nice option for you.  It will probably take a few sessions to achieve a complete result.  Good luck.

Hematomas/bruising are well known to cause subcutaneous fat atrophy. It should only be treated by fat to replace what is lost. The only debate is whether this should be done by fat injections or a dermal-fat graft. There are arguments to be had with either approach as its relatively small size makes its successful correction amenable to either approach. I would recommend an initial fat injection treatment with the understanding that a second procedure is highly likely for optimal results.

There are multiple nonsurgical options to improve this area of indentation cause by traumatic fat atrophy, including soft-tissue fillers. A collagen-stimulating filler (Sculptra) can also be implanted in that area through channels created by a laser device. I hope this helps.

I believe you are correct that the bruise has caused fat atrophy and scar contracture.  The photo suggests that the area is about the size of a golf ball and probably too big to be treated economically with injectable fillers.  Your own body fat can be used to address this type of problem safely and efficiently with a reasonable expectation for success.  Look for a surgeon in your area with experience in fat transfer (fat grafting).  Fat transfer is a minor surgical procedure, often done in an office setting, and is very well tolerated.
