To correct your jowls and sagging mid and lower face region, there are surgical and non-surgical options. The surgical options include lifting the mid and lower face regions with small incisions around your ears. Based on your photos it seems that you are an excellent candidate for lower face and neck lift. This procedure will allow removal of jowls and tightening of lower face and neck to give a definition to a jawline. This is achieved by tightening and lifting deep structures of the neck and face (SMAS). The good news is that the kinds of problems you demonstrate in the photos, which are seen daily by those of us who specialize in aesthetic medicine, i.e. the loss of volume, especially in the infraorbital region and the upper inner portion of the cheek (on the side toward the nose), the deepening smile lines and marionette lines, the incipient jowl formation along the jawline, the loss of volume in the upper lip ( in particular), and the development of "bar code" (aka "smoker's lines" or "lipstick bleeding lines") above the upper lip, and the suggestion of some downturning of the outer third of the eyebrows are ALL easily amenable to the advanced uses of injectable volumizers and fillers. In experienced hands, treatment of all the above should not take more than about 45 minutes without the need for cutting, stitches, or any significant downtime. If good technique is used, this can be done with minimal scarring and can result in a natural look, not over-done look, with rejuvenation of your face. Make sure you specifically look at before and after pictures of real patients who have had this surgery performed by your surgeon and evaluate their results. Hi, I have performed many facelifts over the past 30 years, many of which have been the minimally invasive type SMAS facelifts. Fro the photos, you have nasolabial folds that a very light excess of fat within them which tends to allow gravity to pull them towards the nose and mouth at an accelerated rate. The lower edge of that fold is called "Jowling" or "Jowls". "Jowls” are sagging facial tissues and an indication for some form of a SMAS facelift. The underlying SMAS layer, of the face, must be dissected, lifted, trimmed and re-sutured (not merely folded or suspended with threads or sutures that will not last). The excess skin is then removed and the facelift incisions closed. For well over 15 years now, my most popular facelift is the minimally invasive, short incision facelift that has all the benefits of more invasive facelifts (traditional, mid-face, deep plane, cheek lift and subperiosteal facelifts) but with these added benefits:very small incisions and no incisions extend or are placed within the hair.minimal tissue dissection = less bruising and swelling = rapid recovery ( several days instead of weeks or months with the more invasive type facelifts mentioned)can be performed in 90 minutes or less, with or without general anesthesiano incisions within the hair = no hair lossexcess fat can be removed from the face and neckexcess skin removed from the face and the neckcheeks, chin and jaw line can be augmented with dermal fillers (I prefer Restylane Lyft) or facial implantsmost patients fly back home to parts all over the world in as little as 3 days post-op I combine facial shaping with every facelift procedure. When jowls are present, these should be done in concert and not alone or separately in order to create a naturally, more attractive face. I would place a precise amount of dermal filler below the very narrow, angular sections of your cheeks (outside sections) to soften and further feminize the cheeks and face.
