Looking at implants to help with shape and restoring my upper pole\shape - Help with motiva implant size and profile? (Photos)

Thank you for your question, and history. My first suggestion is to consider that the whole torso needs to be considered in the 'aesthetic unit' when looking at breast size. Therefore, if you have an abdominoplasty, your 'tighter torso' will lower your requirement for size, because you will be in better balance postop with the abdominoplasty. Now, my second suggestion: breast shape is the most important thing and it is quite possible that a repeat mastopexy alone will give you a nicer shape as well as enough upper pole fullness to be in balance and look great. And the added bonus is: no implants. How would you know? I would, at the very least, see a surgeon who has VECTRA imaging and a virtual mastopexy can be done to give you an idea of what a repeat mastopexy alone would look like. It is not a guarantee, but is a good communication tool. If you are near the NYC area, I do this at no charge to the patient. Anyway, best of luck and I hope I helped.

It great that you have done some research about these implants. With respect to the Motiva implant options, if your goal is to achieve and maintain more upper pole fullness, I would recommend the "Round" implant in the "Mini" profile.
The Round implants are more cohesive than the Ergonomix implants, so when you are standing upright, the more cohesive implant is going to maintain that upper pole shape. Regarding the profile, the Mini profile is going to give you more upper pole fullness than the Demi profile. The Mini implants are essentially a 'low' profile implant, and the Demi implants are essentially a 'moderate' profile implant. So what that means is that, for a given volume, the Mini implant is going to have a wider diameter (i.e. the implant it "taller" or "wider"), which translates into more upper pole fullness. Comparatively, a Demi implant is more to have more projection outward than a Mini implant but a slightly narrower diameter, so it's not going to provide quite a much upper pole fullness.
If you are considering doing another lift at the same time, you are going to lose a little bit of volume with the lift. Based on your photos, I wouldn't anticipate losing a lot of volume, because the lift would mainly be to raise the areolas up a little bit and shape the lower pole of the breasts. Taking that into consideration, along with your provided base width measurement of 11.5cm, and not wanting to increase in volume significantly, I think something in the neighborhood of 230cc would be appropriate for you.
The Motiva Round Mini 230cc implant has a diameter of 11.25cm (which is close to your natural base width), so it would fit your chest appropriately. The projection is 2.7cm, so it's not going to be sticking out terribly far from your chest. And the more cohesive "Round" silicone would provide you with more consistent upper pole fullness.
