Based on your measurements and wish photo I would suggest the Mentor Smooth Round Moderate Plus Xtra profile. They give a nice natural look but also would help add the cleavage you are looking for. Always have your surgeon measure your BWD to be as measurements can differ.

breast implant type and sizing selection is a complicated process and requires a detailed consultation that includes a discussion of your goals, a detailed examination, and a lengthy discussion. But the decision will always start and end with the patient. When it comes to sizing, we have all of our patients find 3 photos of their desired breast size. We ask for naked breast photos because clothing can obscure the true size (push up bras). Next, we consult with patients and go over the photos and take detailed breast measurements during a physical exam. Next, we using a sizing algorithm to determine a range of implants that will work based on the exam and the measurements.Lastly, during the operation, we use silicone sizers in the operating room to see how the size actually looks in the breast. We sit the patient up in the operating room, while asleep of course, and compare the sizer result with their desired look photos. If it is too small, we start the process over with a slightly larger implant. We don't stop until we get the perfect match.Of all of my breast augmentation patients, every one of them has been happy with their size due to this detailed and meticulous process. You don't want to invest time and money into a result that is either too big or too small.You should discuss your concerns with a board-certified plastic surgeon and make sure that your surgeon understands your goals.

Thanks for posting your pictures and your goal picture. From observing your pictures you appear to have small A cup size breasts. 250-275 cc implants will bring you to a 34 B+ cup size. Considering your body dimensions, you might want to consider becoming 34 small C cup size (325 - 350 cc) , which is more proportionate to your body and very natural looking. I would recommend a moderate profile Saline implants made by Mentor for multiple reasons. The best surgical approach for obtaining nice and predictable cleavage is via sub areola incision.
Always, consult with experienced board certified plastic surgeons who operate in accredited surgery center for your safety. Most importantly, check the before and after pictures in the photo gallery, to make sure that they are numerous, consistent and attractive with nice cleavage, perky and natural looking. Check also the reviews, to build up confidence in your surgeon.

When selecting an implant it is always best to meet in person with your board certified plastic surgeon. She/He will measure your breasts, discuss your goals, for surgery, and assist in selecting the right implant to help meet those goals. For a more natural look with conservatively sized implants you may consider a moderate plus profile implant. I've included a video on implant selection that I hope you find helpful.

Measurements are needed to determine the best range of size for your tissue and frame. However, the range you are suggesting should look very nice and natural for you. Probably the moderate plus profile would work well.
