My nostrils are asymmetrical following an open septoplasty (without rhinoplasty). I had no obvious asymmetry before and liked the appearance of my nose. I'm so upset. What can be done? Does it require a full on rhinoplasty with full sedation or is there anything non-surgical?

Thank you for your question and for sharing your photos. Nostril asymmetry after septoplasty can result from several factors, including caudal septal position, scar contracture, swelling, or changes in the support and position of the nasal tip and nostrils.
Timing matters. If surgery was recent, continued healing and scar maturation may improve some asymmetry. If the asymmetry persists and is structural, non-surgical treatments (such as filler) generally have a very limited role if any.
Importantly, correction does not necessarily require a full cosmetic rhinoplasty. Depending on the underlying cause on preoperative exam and during intraoperative assessment, a focused revision may be able to restore tip symmetry and support while preserving the parts of the nose you already like.
An in-person examination with a plastic surgeon experienced in revision rhinoplasty and functional nasal surgery is the best way to determine the cause and appropriate treatment.
