I had traditional lipo on my thighs about 10 years ago. I'm now in my mid 30's. In a surgeons opinion can repeat surgery help the appearance or make it worse?

Answer: Revisionary liposuction to the thighs can be tricky
You appear to have some fascial laxity globally along your thighs - the skin is not tightly approximated to the underlying soft tissues. I would be reluctant to offer revisionary liposuction in your case.

Answer: Can Repeat Lipo Make Legs Look Worse?
I may not recommend any surgery at all, but you may be a candidate for liposuction with fat grafting. I would have to examine you in person to know whether or not that would help. Find a plastic surgeon with ELITE credentials who performs hundreds of liposuction and liposuction revision procedures each year. You have a difficult problem. Obviously, issues like this will be hard to comment on without a full examination and standardised set of planning photos.
From your photo it appears that the main issue that you have is contour problems with divots in some areas and lumps in others. The overlying skin does not appear to be tight on the underlying fat at the moment. I suspect that your initial problem may have been cellulite, which can end up this way after liposuction.
Minimally invasive machines won't work in your scenario, but there will be people that want you to spend money on their expensive machine so they can pay it off.
Additional liposuction may help, but there will be a risk that it will be made worse. The addition of additional energy (such as ultrasound with VASER liposuction) way also help, but probably increases the risk of additional problems. Your best bet may be liposuction with fat equalisation (or small fat grafts).
I doubt that you will ever be fully satisfied with any revisionary procedure, but it can be made better.
