Do I need a chin implant or maxillofacial surgery and will it help erase the wrinkles on my chin? (photo)

A good exam may be required before one can adequately answer this question. It may depend on your occlusion of your teeth. If your goal is to improve your profile, an implant or a sliding genioplasty may suffice and discuss with your surgeon the pros and cons of each technique. If your only concern are the wrinkles, you may wish to also consider Botox treatments which can be of use with this.

Thank you for your pictures.  I would recommend a sliding genioplasty to enhance your chin.  The "wrinkles" will improve because they are produced by straining in your mentalis muscle.  

To adequately eliminate your mentalis strain, you need a sliding genioplasty. This will provide good chin augmentation and will eliminate the abnormal motions of the mentalis muscle. These wrinkles are the mentalis muscle straining to achieve competent lip closure.

 A chin implant will not help with wrinkles of the mentalis muscle. The wrinkles are as a result of your jaw/chin being too small. I believe you should be seen by a surgeon, take X-rays and find out which treatments is best for you. 

 Hi, from the attached photo the chin is very weak.  Chin augmentation, with a chin implant, can provide harmony and balance to the lower face.  The "cobblestone" appearance in the skin is from an over-active mentalis muscle.  The chin implant is placed below the muscle and will not stop that effect.  Injections of Botox or one of the other neuro-modulators will selectively weaken the muscle reducing the unwanted effect.  Hope this helps.
