Without an exam your question cannot be answered.  You will need to see an Oral/Maxillofacial surgeon to see if othognathic surgery, genioplasty or using an implant is best for your specific case. It should be noted that patients with severely weak jaws or abnormally functioning jaws may be better treated with other facial surgery (orthognathic surgery), although a chin augmentation may be an option at a later time. Hello. I agree with your description of form and proportions as well as with your ideals. As you well expect, the ideal under-chin should be close to a 90 degree angle that forms with your neck. It is not always FAT that causes the double chin; it can be muscle laxity. In my professional opinion, you would most benefit from a submentoplasty and chin implant placement. During these procedures, the same under-chin incision is used to remove excess fat and Tightening of the platisma muscles under your chin as well as chin implant placement. The result is a more prominent chin and a well defined under chin area (submental area). 

A chin implant or a sliding genioplasty are both excellent options for improving one's profile if one has a week chin. Shin implants tend to be more common as they are relatively easy to place. Most models are also easy to remove if one does not like the effect. There is always a risk for an infection if a foreign body is placed although this tends to be not common. A sliding genioplasty in the right candidate can be appropriate.

 A sliding genioplasty is performed by oral surgeon under general anesthesia as an inpatient procedure a hospital, with an overnight stay. A chin implant is performed for cosmetic purposes under local anesthesia as an outpatient procedure. Chin implants are manufactured in a variety of sizes and shapes, are composed of Silastic, and can be custom carved when necessary to fit the patient's specific  anatomical requirements. The fatty deposits( submental and sub-platysmal) in the neck will also require some degree of  removal, which may include a platysma plasty to give a sharp angulated jawline.

While both a chin implant and a sliding genioplasty will provide significant improvement I would leans towards the sliding genioplasty because it has greater neck benefits and can vertically lengthen your chin as well. Also some neck treatment will be needed as well such a liposuction or a submentoplasty to try and get the neck angle closer to 90 degrees. ideally a opening sliding genioplasty with a submentoplasty is needed to get you closer to your ideal chin/neck angle change. A chin implant and neck liposuction will help but will not provide the same degree of improvement.

  Hi, based upon the attached photos your chin is quite weak and as such it does appear that you are a good candidate for chin augmentation with a chin implant.  We have performed many chin augmentations using chin implants to provide harmony and balance to the lower face.  The excess fat and skin, under the chin, can be removed at the same time as the chin implant surgery.  If further jaw line definition and flare is desired, dermal filler (I prefer Radiesse) can be placed along the mandibular angle to create more angularity to the face which is perceived as ruggedly handsome.
