Advice for mini boob job, small implant, 150cc? (photo)

Question

I am 5’4”, 117lbs athletic build. I am desiring a very natural look, considering a small around 150cc mini breast augmentation. I cannot find too many women who have had such small implants. I have rib flare and would like to have a bit more projection than my ribs but I don’t know that I like how the profile (MP, HP) implants look.

The best appproach to achieve your results start with a consultation at a board certified plastic surgery office. Different sized implants with various projections  can then be discussed with you and the provider.We use the Vectra 3D imaging to estimate the size and shape that closely matches your preference. Also before and after photos of other patients are helpful in determining the final implant selection.And I would recommend the implant placed below the muscle if less than 200 cc in size.

Depending on the individual anatomy and desired results, estimations and measurements should be made for each patient individually. The reality is that you are starting with tiny breasts and a tight skin envelope,  but you want a noticeable difference to your breasts with the implant selected. Most patients with small breasts are better served by a submuscular augmentation to permit a more natural breast takeoff and better camouflage for the implant.  A board certified plastic surgeon will be able to guide you to the correct size implant for your measurements and desires.  Measurements of the chest wall and breast imprint width as well as knowledge of all breast implant sizes and profiles should make this process easy.  With low and moderate profile implant sizers the PS recommends, determine how they look under a shirt, workout clothes, and other garments you typically wear so that you have the best idea of whether the garments fit as desired with the implant selected. 
I would recommend that you go with the size that boosts your self-esteem, and fits your anatomy.  Choosing an implant based on an aesthetic goal that exceeds your anatomic capacity may lead to complications and possibly revision. Based upon his/her credentials, trust your surgeon to make the best decision in the OR when the implants are placed.

Your photo indicates a small-breasted, slender female with minimal body fat.  The 150cc implant size you mentioned will give you a very slight size increase, perhaps less than or just equal to a full cup size up.  In addition to the size choice, consider the profile which correlates to the degree of projection.  Due to your minimal breast volume and body fat, almost any implant will show more of a defined breast border rather than a more gentle slope.  A low profile implant is flatter and wider for a given size than the moderate or high profiles, so it may look more natural.  Be sure to thoroughly discuss with your surgeon the relative appearance of the different profiles in your chest wall.

Small implants in the 150 cc size have a diameter around 9 cm.  As long as your breast width at the base is around this width, then it should be fine.  You can try these implants out in the office to see if it gives you the projection you are looking for.

Thank you for your pictures and questions. There are not many patients like you out there, but they do exist from time to time. I provided a picture of a case I have done similar to yours. Athletic, thin, wanting a natural result. I know that textured implants have been getting a bad rap in the media lately but a case like yours with your goals screams "shaped textured implant" to me. Choose a shaped and wide oval base with modest projection (low/moderate) in a low volume, probably something around 200 cc's and you will probably really love what you see. I would recommend seeing a plastic surgeon who uses 3D imaging to show you what the results might look like. You need a formal evaluation by a licensed plastic surgeon and expert in breast augmentations to determine what is best for you and your goals.
Proper breast enlargement is not a "cookie-cutter" procedure where the same technique is applied to every patient. Every woman who comes to my practice has unique needs and different goals for her breast augmentation. I also take into account their overall height and weight, the size and shape of the underlying chest and ribcage, the tightness and elasticity of the skin, and the amount of natural breast tissue. The size, profile, and shape of the implant is based on your desired breast size/shape, your chest wall measurements, and soft tissue quality. This decision should be based on a detailed discussion with equal input from both you and your surgeon. Make sure you specifically look at before and after pictures of real patients who have had this surgery performed by your surgeon and evaluate their results.
