I've lost quite a bit of weigh since my initial consultation, and my PS and I settled for Sientra smooth round LOW profile 250cc implants. after researching so much I'm scared that I will have a wide flat boob and won't give me the look i want. I want more upper pole fullness and not super fake. Im not sure if I need to switch my implant profile. My BWD was 13.9 back in June so I'm not sure if it's less now since I've lost weight.

Answer: What breast implant profile should I choose? The answer to which breast implant profile you should choose is both very simple, and yet very complicated. Simple answer: the one that looks the best and gives you the volume that you want. The more complicated answer is the one that fits your base diameter AND gives you the volume that you want. This is where things get complicated because plastic surgeons have all kinds of different methods and techniques for determining base width diameter; some don't take any measurements at all. This is where I have a fundamental difference of opinion with ALL other plastic surgeons. Based on the way I measure my patients there is absolutely no way that your base width is 13.9 and using an implant that wide will just widen your breast and make it push into the armpit. Having said that, I'm sure that there are plastic surgeons that would claim that your base width is 15 and encourage you to use an even wider implant.Bottom line, I know the plastic surgeon you are seeing and you are in good hands. He's a good doctor. If you like the way his breast augmentation patients look then you will likely achieve similar results. If you want to learn more about breast augmentation you can download a free breast augmentation planner at aBetterBreast dot comRichard H Fryer, MD FACS Board Certified by American Board of Plastic Surgeons (ABPS) member American Society of Aesthetic Plastic Surgery (ASAPS) member American Society of Plastic Surgery (ASPS)

Answer: Should I get low profile or moderate? Other? Hello and thank you for your question and sharing your photos. An in-person examination is required to give you a useful advice. Implant profile to a large extent is based on your desired breast volume and the width of your breast footprint. The width of you breast footprint is a constant number and will not change with fluctuations in your weight. Most plastic surgeons use many modalities including in-bra sizers and goal pictures to select the final implant. Your final result depends on lots of things including your chest wall anatomy, skin envelope, and degree of breast ptosis among others. My best advice is that you communicate your goals and aesthetic ideals with your surgeon as clearly as possible and trust your surgeon's skills and abilities to deliver what you are looking for. Best wishes and good luck, Pejman Aflaki, M.D. Johns Hopkins-trained double board-certified plastic surgeon
