Hi! I fell a few years ago on a metal ledge and damaged my butt. I ended up with a slight divot across the midline, more-so the left cheek. What procedures can be done to help with this? I feel like it's made my butt sag & and I've lost the nice projection I had previously. I even feel like it's a dead spot and get a sore sensation in the area occasionally. Looking for safe procedures and recommendations for doctors. Willing to travel.

Sounds like a terrible accident. By examining your photos, the dents look like they can be corrected with fat transfer. The issue then becomes where to find the fat on your body. You don't seem to have too much fat of the abdomen and back. However, I don't think filling these dents in would take too much fat. You could consider VASER liposuction of the abdomen, back, and arms to give you a toned look, and then the suctioned fat will be washed with antibiotics and transferred into the dents. If you choose this option and would like to have more buttock projection, a doctor may suggest that you gain 10-20 lbs. Sculptra is an FDA-approved injection that promotes collagen growth. However, it's quite pricey and will likely take multiple treatments. There's also a risk that the specific buttock dents will not be properly targeted. Therefore, I recommend VASER liposuction with fat transfer. I do a lot of reconstruction + BBL after silicone removal and that includes filling the dents in the buttocks. However, this area tends to be quite resistant to this technique. So if tried and failed, i have actually had the best success popping out dents of all sorts using buttock implants. The only caveat is, you must be willing to also augment your entire buttock a small to moderate amount during this process. Please note, an extremely few number of board certified plastic surgeons specialize in buttock implants, so you may need to travel far and away from home to find this particular surgeon. Answer: Fixing trauma induces "dents" on the buns or elsewhere Hi. My recommendation to fix the indentation caused by your fall is a fat transfer. I have had really good success with this in previous patients. I usually take ("harvest") the fat from the abdomen (best fat for transfers). With my fat transfers, I will do the first transfer at the time of the fat harvest. This is when there are the most live fat cells for a true graft. We then store the fat in a regulated freezer system to be used for as many transfers as you want in that time period. Most patients do a series of 3 to 4 transfers. I've seen it stay in place for at least 10 years - for traumatic indentations.
