Great question. Thank you for your pictures. You mention you want to avoid a scar. I will suggest perhaps getting skin tightening (BodyTite) for some improvement. Please visit a board certified plastic surgeon for a detailed surgical plan to achieve your desired results,.

In order to improve your final results, I also recommend adding vanquish for further fat removal, exilis/venus legacy for skin tightening, cellutone/z wave to make the skin smooth, remove scar tissue, and improve the skin’s appearance. See an expert. Best, Dr. Emer

it is hard to tell for sure without an examination, but based on your photos and description it looks like this can be improved with another fat transfer. If you are considering a surgery, I would suggest you to consult a board-certified plastic surgeon. Only after a thorough examination you will get more information and recommendations.

To redefine your infragluteal folds, a lower buttock tuck/lift is needed. Trying to fill out the low folds that is associated with loose skin is not going to work. Besides poor fat take this would elongate your buttocks even if it survived. The loose skin must be removed and tucked up and in to get higher more defined infragluteal folds.

it's important to understand the primary problem is that volume was removed.  The correct way to treat this is to restore the volume.  It is difficult to get enough fat to survive in a small area to compensate for the amount of fat removed from your liposuction.  Cutting out the skin will most likely not leave you with a good result. If you think your previous fat transfer gave some improvement then a second fat transfer may be the way to go. If you were consulting with me I would've told you that you will need several rounds of fat transfer to get the results and even then there are no guarantees.  Best, Mats Hagstrom MD
