Question

I have an open bite and a recessed chin I would like to fix because it's causing a protrusion . Also it looks as if my neck and jaw run together. How to do I achieve a side profile like the second picture

You may consider a chin augmentation to improve your jawline combined with liposuction of the sub mental chin and neck. Liposuction of the chin area alone could give improvement as well. I would recommend an in person consultation with a Board Certified Plastic Surgeon.

I suggest beginning with an in-person consultation to discuss your options. During your initial examination, your #chin will be evaluated along with the neck and jawline. A weak #chin implies a lack of chin support for the skin along the neck line. In younger individuals a chin implant alone is usually all that is needed to improve the appearance of the neck. Subsequently, it will also improve the overall #balance of the face and can create the illusion of a smaller nose. As a stand-alone procedure, chin augmentation can be performed in the office under local #anesthesia. For individuals with signs of #aging in the neck, a chin implant added to one of the recommended procedures above, will provide optimal results. Also, #liposuction under the chin is also another option your doctor can discuss. If you are not a candidate for chin or neck rejuvenation at the time of your #consultation, your board certified plastic surgeon can recommend alternatives to address your concern. 
However, there may be additional options to improve the appearance of your concern to improve the overall balance of your face. Such can be best evaluated in-person. Consulting with a board-certified plastic surgeon who specializes in #face, #neck and #chin procedures is important to ensure you are receiving the best advice, care, and surgical experience possible.

Based on your photo you are a good candidate for chin liposuction,  Chin liposuction may be an option, it makes you look younger and more attractive with a sharp jawline , and better facial proportions.
I recommend you to make an appointment with a board certified plastic  surgeon for a good evaluation and to talk about your goals.

Hi, I have performed many facial shaping procedures, including Chin Augmentation with dermal fillers or silastic chin implants, for over 30 years.  From the photo, your chin is quite weak as are the mandibular angles.   When the chin is weak, this creates an imbalance making the nose appear larger, the mid face top heavy, the lower face looks short, de-emphasizes the lips and allows early formation of a "double chin".  
 Proper placement of a silastic chin implant adds forward projection to the chin thereby creating harmony and balance to the lower face.  Using the same incision, liposuction can be performed to reduce the fat and further shape the neck.  Excess skin, from below the chin, can also be removed through the same incision.  
 I have found that placement of a silastic chin implant, through a small curved incision under the chin (also allows excess skin removal) to be very safe, quick, highly effective and far less invasive than a sliding genioplasty (requires extensive tissue dissection, bone cuts and placement of metal screws and plates to secure the cut segments of bone).  I perform chin implant surgery in 30 minutes or less, often using a local anesthetic alone.  In my opinion, you are a good candidate for chin implant surgery.   Augmentation of the mandibular angles using a dermal filler (jaw implants have too many unwanted side effects IMHO) to add volume, angularity and flare to the jaw line.Hope this helps.
