I would suggest scheduling a consultation with a board-certified plastic surgeon in your area who provides Mommy Makeovers and speaking with them about what they typically include in the procedure. Your surgeon may suggest losing a few more pounds naturally before getting surgery. I think that those goals can likely be achieved with various mommy makeover techniques as you have suggested.

Based on your desires and photos, I think a breast reduction (which includes lifting and reshaping the breasts) and a standard tummy tuck would help most.  A small/moderate sized breast implant could be considered after discussion with your surgeon, only for the purpose of increasing firmness and upper breast fullness in the cleavage area.  This combined surgery is labeled a mommy makeover.  Adding liposuction to other areas of the trunk and/or the extremities will add hours to an already long surgery under anesthesia.  If you are in excellent health and the surgeon has a good assistant to help, this could all be done together, although I would think it is a theoretically safer to do this in two separate procedures.
